Nicotine transfers into breast milk quickly and cleanly. The concentration in milk runs roughly three times the concentration in maternal plasma, and it peaks about 30-60 minutes after a vape or cigarette. The half-life in milk mirrors plasma at roughly 90 minutes.
That sounds alarming and it does matter — but the recommendation from the American Academy of Pediatrics is unambiguous: continue breastfeeding even if you use nicotine. The immunological, gut, and developmental benefits of breast milk outweigh the exposure.
Why breastfeeding still wins
- Breast milk transfers antibodies formula cannot.
- Formula infants have higher rates of respiratory illness, and adding a nicotine-using parent's household to the equation increases that risk further.
- The absolute nicotine dose transferred is small compared with in-utero exposure.
If you can quit
That is the goal. Postpartum is the highest-relapse window in a nicotine-user's life: sleep deprivation, hormonal shifts, and the disappearance of the "I'm doing it for the baby" motivator all pull the wrong direction. See postpartum relapse prevention.
If you cannot quit yet
Two rules matter more than anything else:
- Never vape in the same room as the baby. Secondhand aerosol contains nicotine, ultrafine particles, and flavouring compounds. Thirdhand residue coats surfaces and clothing.
- Time feeds to the trough. Vape immediately after a feed, not before. Wait 2-3 hours between a vape and the next feed if the schedule allows.
NRT while nursing
A nicotine patch is generally preferred to continued vaping while breastfeeding: steady low dose, no aerosol carrier, no peaks that spike milk levels. Removing the patch at bedtime lowers the overnight level for the pre-dawn feed. This is a paediatrician conversation.
What not to do
- Do not switch to formula because of nicotine — the AAP is clear this is the wrong trade.
- Do not "pump and dump" to speed clearance — nicotine clears at its own rate whether or not you pump.
- Do not hide use from your paediatrician — they can only help you quit if they know.
