Nicotine gum is the most-used short-acting NRT form. It works in about 10-15 minutes — roughly the arc of a real craving.

Choosing your strength

  • 2 mg: light or moderate vape use (~10-20 mg/day nicotine).
  • 4 mg: heavy use (Juul-a-day, one 20-50 mg/mL disposable a day, or first cigarette within 30 minutes of waking).

If in doubt, start at 4 mg and see how it feels; drop to 2 mg if you get side effects.

The chew-and-park technique

  1. Chew slowly, once or twice.
  2. Notice a peppery or tingly taste.
  3. Park the gum against the inside of your cheek.
  4. Wait until the taste fades (~1 minute).
  5. Chew a couple more times to re-release.
  6. Repeat for about 30 minutes.
  7. Discard.

Chewing gum like regular gum sends the nicotine down your throat, which causes hiccups, heartburn, and no effect.

Timing

  • Use as soon as you feel a craving.
  • Or use preemptively before a known trigger (a meeting, a drive, a bar).
  • Do not eat or drink coffee, tea, orange juice, or soda 15 minutes before or during use — the acid blocks absorption.
  • Typical schedule in the first two weeks: one piece every 1-2 hours while awake; taper down after week 4.

Side effects

  • Hiccups, jaw ache, mouth soreness (usually from chewing too fast).
  • Nausea (usually from too high a dose or missed technique).
  • Not addictive at the doses used, but continued long-term use maintains mild dependence.

Combining with a patch

Pair a 21 mg patch with 2-4 mg gum for combination NRT — the highest-effect NRT protocol in Cochrane data. See combining NRT.

When to switch to something else

If you cannot tolerate gum (jaw problems, dentures), try lozenges or spray.
