When a quitline or text program isn't enough, human coaching is the next step up.
One-on-one quit coaches
- How it works: weekly or biweekly 30-45 minute sessions, phone or video, over 8-16 weeks.
- Access: most US private insurance and Medicaid cover it at zero copay under the Affordable Care Act. Ask your plan for "tobacco cessation counselling".
- Employers: many wellness programs include unlimited coach access; some offer cash incentives ($100-500) for milestones.
Freshstart (American Cancer Society)
- Four one-hour group sessions over four weeks.
- Structured curriculum: preparing to quit, quit day, staying quit, relapse prevention.
- Free, run at hospitals, workplaces, and community centres.
- Search "Freshstart" on cancer.org for local sessions.
Nicotine Anonymous
- 12-step peer group, in-person and virtual meetings.
- Free, anonymous, not clinician-led.
- Works for people who liked AA-style community and want ongoing support past the acute quit phase.
Workplace wellness programs
Common features:
- Coach access.
- Free NRT.
- Cash incentives.
- Confidentiality via a third-party vendor (Rally, Virgin Pulse, Quit Genius).

Community and healthcare-system programs
- Local health department clinics.
- Federally qualified health centres.
- University counselling centres.
- Veterans Affairs tobacco cessation program.
Does group work as well as one-on-one?
Cochrane's review found group counselling roughly matches one-on-one for quit rates. Cost and access differ — group is cheaper; one-on-one is more flexible. Pick what you'll actually attend.
Stacking with medication
Human coaching combined with NRT or prescription medication is the highest-evidence combination outside of research settings.
