Hookah gets marketed and defended as "just flavoured water vapour". It is not. It is combusted tobacco, and the smoke is real smoke.
The mechanics
Hookah heats tobacco (usually shisha-blended with molasses and flavouring) using charcoal. Smoke is drawn through a water chamber into a hose. The water cools the smoke; that is what makes long sessions bearable, not safer.
What a session actually delivers
A 45-60 minute hookah session:
- Delivers ~90,000 mL of smoke, vs ~500-600 mL for a single cigarette.
- Produces high carbon monoxide because of the charcoal.
- Delivers nicotine comparable to several cigarettes.
- Includes all the combustion toxicants smoking does: tar, particulate matter, tobacco-specific nitrosamines.
Vaping produces none of those combustion products.
Where vaping wins
- No combustion, so no CO, no tar, no combustion-specific carcinogens.
- Smaller per-session toxicant load.
Where hookah wins
Nothing quantifiable. The "social ritual" appeal is real; the health defence is not.
Both share
Nicotine dependence. Whether from a hookah bowl or a Juul pod, the addiction is the same molecule.
If you do both
Prioritise stopping hookah first, the per-session exposure is much higher. Then handle the vape dependence with NRT or approved medications.
